Testosterone replacement therapy, commonly called TRT, should never be a “prescribe it and forget it” treatment.

Safe and effective TRT requires scheduled laboratory testing, blood pressure monitoring, symptom reviews and appropriate prostate screening. These checkpoints help your provider determine whether treatment is improving how you feel without pushing testosterone, hematocrit, blood pressure or other health markers in the wrong direction.

The goal is not to produce the highest testosterone number possible. The goal is to restore testosterone to an appropriate physiological range, improve symptoms and manage risk over the long term.

Current guidance continues to emphasize that TRT should begin only after an accurate diagnosis and should include ongoing monitoring. In July 2026, the Endocrine Society reiterated that hypogonadism requires both consistent symptoms and at least two low early-morning testosterone results. The organization also stressed that prostate risk assessment and continued monitoring remain important because long-term safety questions have not been completely resolved.

Here is what a responsible TRT monitoring schedule typically looks like.

This is a general framework, not a rigid protocol for every patient.

Some patients need earlier or more frequent testing. This may include men with elevated baseline hematocrit, sleep apnea, obesity, cardiovascular disease, prostate concerns, significant urinary symptoms or recent changes to their testosterone dose or delivery method.

Why TRT Monitoring Matters

Testosterone affects more than libido and muscle mass.

It can influence:

  • Red blood cell production
  • Blood pressure
  • Fluid balance
  • Prostate tissue
  • Fertility
  • Sleep
  • Mood
  • Sexual function
  • Metabolic health
  • Medication response

TRT monitoring helps answer four basic questions:

  1. Is the treatment working?
  2. Is the dose appropriate?
  3. Are side effects developing?
  4. Does the original diagnosis still make sense?

A laboratory result should never be interpreted in isolation. Your provider should compare your numbers with your symptoms, health history, medication timing and treatment goals.

Someone can have an impressive testosterone result and still have poor sleep, uncontrolled blood pressure, elevated hematocrit or no meaningful symptom improvement. That is not successful hormone optimization. It is simply a satisfying-looking number on a report.

What Should Be Checked Before Starting TRT?

A safe monitoring schedule begins before the first dose.

Confirm the Diagnosis

Symptoms such as fatigue, low libido, reduced motivation, erectile dysfunction and poor recovery can occur with low testosterone. They can also occur with sleep apnea, thyroid disorders, depression, obesity, insulin resistance, medication use and chronic stress.

That is why symptoms alone are not enough.

The Endocrine Society recommends confirming hypogonadism with symptoms plus consistently low testosterone concentrations. A low result should generally be confirmed with a second morning fasting testosterone measurement using an accurate laboratory method.

Baseline hormone testing may include:

  • Total testosterone
  • Free testosterone
  • Sex hormone-binding globulin, or SHBG
  • Luteinizing hormone, or LH
  • Follicle-stimulating hormone, or FSH
  • Prolactin when clinically appropriate
  • Estradiol when indicated

LH and FSH can help determine whether low testosterone is primarily related to testicular function or signaling from the pituitary and hypothalamus.

Complete Blood Count and Hematocrit

A complete blood count, or CBC, measures hemoglobin, hematocrit and other blood cell markers.

Hematocrit represents the percentage of your blood volume made up of red blood cells. Testosterone can stimulate red blood cell production. If hematocrit rises too high, the blood may become more viscous.

A high baseline hematocrit deserves evaluation before TRT begins. Potential contributors may include:

  • Untreated obstructive sleep apnea
  • Smoking
  • Chronic lung disease
  • Dehydration
  • High-altitude residence
  • Existing blood disorders

European Association of Urology guidance identifies a baseline hematocrit between 48% and 50% as a reason for additional caution. A hematocrit of 54% or higher represents a major safety concern.

Blood Pressure

Blood pressure should be measured before treatment begins.

This has become even more important following class-wide testosterone labeling changes. In February 2025, the U.S. Food and Drug Administration reported that required ambulatory monitoring studies confirmed blood pressure increases across testosterone products and required updated warnings.

Baseline blood pressure provides a comparison point for future readings. It also helps identify untreated hypertension before therapy is introduced.

PSA and Prostate Risk

Prostate-specific antigen, or PSA, is a blood marker produced by prostate tissue. PSA is not a stand-alone cancer diagnosis. It is one part of evaluating prostate health and monitoring changes over time.

Before starting TRT, prostate screening decisions should consider:

  • Age
  • Baseline PSA
  • Family history
  • Personal history
  • Race and prostate cancer risk
  • Urinary symptoms
  • Previous prostate biopsy or treatment
  • Shared decision-making preferences

The Endocrine Society recommends discussing prostate risk assessment with appropriate men before treatment and repeating that assessment within 3 to 12 months after TRT begins.

Metabolic and Cardiovascular Health

Depending on the patient’s history, baseline testing may also include:

  • Comprehensive metabolic panel
  • Lipid panel
  • Hemoglobin A1C
  • Fasting glucose
  • Fasting insulin
  • Thyroid markers
  • Apolipoprotein B
  • Lipoprotein(a)
  • Waist circumference
  • Body weight
  • Cardiovascular risk assessment

TRT should not replace treatment for poor sleep, uncontrolled blood sugar, high blood pressure, inactivity or inadequate nutrition.

Fertility Goals

TRT can suppress LH and FSH signaling, which may reduce sperm production.

Men who are trying to conceive now or may want children in the near future should discuss fertility before beginning testosterone. That conversation becomes considerably less convenient after sperm production has already dropped.

Other medical options may be more appropriate when maintaining fertility is a priority.

The First Follow-Up: Approximately 4 to 8 Weeks

An early follow-up is often scheduled within 4 to 8 weeks after TRT begins or after a meaningful dosage change.

This visit evaluates whether the treatment is producing an appropriate response and whether early side effects are appearing.

Testosterone Levels

Follow-up testing may include:

  • Total testosterone
  • Free testosterone
  • SHBG
  • Estradiol when clinically indicated

The correct timing of the blood draw depends on the formulation, dosage schedule and clinical question.

A level measured shortly after an injection may represent a peak. A result measured before the next dose may represent a trough. Gels, pellets, oral formulations and longer-acting injections each have different testing considerations.

The most important rule is consistency.

Your provider should specify when the blood should be drawn relative to your most recent dose. Random testing at different points in the dosing cycle can create misleading comparisons and unnecessary dosage changes.

Complete Blood Count and Hematocrit

Hematocrit should be reassessed during early treatment, especially in patients using injectable testosterone or those with risk factors for increased red blood cell production.

A rising hematocrit may lead the provider to consider:

  • Reducing the testosterone dose
  • Changing the dosing frequency
  • Switching the delivery method
  • Evaluating for sleep apnea
  • Reviewing smoking status
  • Reviewing hydration and altitude exposure
  • Temporarily holding treatment
  • Medically supervised phlebotomy when appropriate

Patients should not treat an elevated hematocrit by repeatedly donating blood without addressing why it is increasing. Blood donation may lower the result temporarily while the underlying dosage, sleep or respiratory problem continues.

Blood Pressure

Blood pressure should be checked during the early follow-up.

If office readings are elevated or inconsistent, home monitoring can provide a more useful pattern. A practical home protocol includes taking two readings one minute apart in the morning and evening for three to seven days.

Before measuring:

  • Avoid caffeine, smoking and exercise for 30 minutes
  • Empty your bladder
  • Sit quietly for five minutes
  • Keep your feet flat on the floor
  • Support your back
  • Keep the cuffed arm supported at heart level
  • Do not talk during the measurement

The American Medical Association recommends recording two readings one minute apart in the morning and evening, ideally for seven days, and sharing the complete log with the care team.

Symptom and Side-Effect Review

The follow-up visit should not be limited to laboratory values.

Your provider should ask about:

  • Energy
  • Libido
  • Morning erections
  • Erectile function
  • Mood
  • Motivation
  • Sleep quality
  • Exercise recovery
  • Strength
  • Acne or oily skin
  • Hair changes
  • Breast tenderness
  • Fluid retention
  • Headaches
  • Irritability
  • Urinary frequency or weak stream
  • Snoring or worsening sleep apnea symptoms

TRT should improve clinically meaningful symptoms. A dose that produces high testosterone but worsens blood pressure, sleep, mood or urinary symptoms is not optimized.

The 3-Month TRT Follow-Up

The three-month visit is an important formal checkpoint.

The European Association of Urology monitoring table includes assessments of symptoms, waist circumference, blood pressure, PSA, hematocrit and testosterone around three months after treatment begins.

At this point, the provider should review:

  • Whether testosterone levels are within the intended physiological range
  • Whether symptoms have improved
  • Whether hematocrit is rising
  • Whether blood pressure has changed
  • Whether PSA monitoring is appropriate
  • Whether new urinary symptoms have developed
  • Whether the treatment schedule is practical
  • Whether the patient is using the medication correctly
  • Whether the benefits still outweigh the risks

Dose changes should be based on the complete clinical picture, not a desire to push testosterone toward the upper edge of a laboratory range.

The 6-Month TRT Follow-Up

Not every stable patient needs every laboratory marker repeated at exactly six months. However, six-month testing may be appropriate when:

  • The dose has changed
  • Testosterone levels remain unstable
  • Hematocrit is increasing
  • Blood pressure is elevated
  • The patient has significant cardiovascular risk
  • Symptoms have not improved
  • Side effects are present
  • The patient has a history of prostate cancer
  • Medication adherence is uncertain
  • The delivery method has changed

The European Association of Urology recommends testosterone and hematocrit checks at three, six and 12 months during the first year, followed by annual monitoring. Higher-risk patients may require hematocrit testing every three months during the first year.

The 12-Month TRT Review

The 12-month visit should be more comprehensive than a quick refill appointment.

A yearly review may include:

  • Total testosterone
  • Free testosterone when needed
  • SHBG when clinically relevant
  • CBC with hemoglobin and hematocrit
  • PSA based on age, risk and shared decision-making
  • Blood pressure
  • Lipid panel
  • Glucose or hemoglobin A1C
  • Comprehensive metabolic panel when appropriate
  • Weight and waist circumference
  • Sleep and sleep apnea risk
  • Sexual function
  • Mood and cognitive symptoms
  • Medication adherence
  • Fertility goals
  • Urinary symptoms
  • Cardiovascular symptoms
  • Treatment benefits and side effects

The provider should also determine whether continuing TRT still makes sense.

Treatment should not continue automatically because the patient has been using it for a year. The decision should be based on objective safety data and meaningful clinical benefit.

How Often Are Labs Needed Once TRT Is Stable?

Once the dose, symptoms and safety markers are stable, many patients transition to testing every 6 to 12 months.

Annual monitoring should generally represent the minimum for a stable patient. More frequent follow-up may be needed for:

  • Elevated or rapidly rising hematocrit
  • Elevated blood pressure
  • Significant obesity
  • Obstructive sleep apnea
  • Smoking
  • Chronic lung disease
  • Cardiovascular disease
  • Previous blood clots
  • Prostate cancer history
  • Worsening urinary symptoms
  • Changing testosterone formulations
  • Recent dose adjustments
  • Inconsistent symptom response

The European Association of Urology recommends annual testosterone and hematocrit monitoring after the first year, with closer monitoring for patients at increased risk of elevated hematocrit.

Which TRT Labs Matter Most?

Total Testosterone

Total testosterone measures both protein-bound and unbound testosterone.

It helps determine whether treatment is producing an appropriate overall level. However, total testosterone alone may not explain the patient’s response.

Free Testosterone

Free testosterone represents the fraction not tightly bound to SHBG.

It may be particularly useful when total testosterone does not match the patient’s symptoms or when SHBG is unusually high or low.

SHBG

Sex hormone-binding globulin affects how much testosterone remains available to tissues.

SHBG can be influenced by age, obesity, insulin resistance, liver function, thyroid conditions and certain medications.

Hemoglobin and Hematocrit

These markers help identify excessive red blood cell production.

A hematocrit above 54% requires prompt medical review. Current European guidance recommends dosage adjustment or withdrawal, evaluation of contributing factors and venesection or phlebotomy when clinically required.

PSA

PSA monitoring helps track changes in prostate activity.

During the first 12 months, the Endocrine Society recommends urological consultation when there is:

  • A confirmed PSA increase greater than 1.4 ng/mL above baseline
  • A confirmed PSA above 4.0 ng/mL
  • An abnormality found during a prostate examination

After the first year, monitoring should follow age-appropriate and risk-based prostate cancer screening guidance.

These thresholds do not automatically mean prostate cancer is present. They mean further evaluation may be needed.

Estradiol

Testosterone can convert into estradiol through the aromatase enzyme.

Estradiol should be interpreted alongside symptoms. Your provider may check it when there is breast tenderness, fluid retention, changes in libido, mood changes or other signs that warrant evaluation.

An elevated estradiol result does not automatically require an aromatase inhibitor. Treating a laboratory value without considering symptoms can push estradiol too low, which may negatively affect sexual function, mood and bone health.

Lipids and Glucose Markers

A lipid panel and glucose-related testing provide information about broader metabolic and cardiovascular health.

European guidance considers baseline and annual evaluation of the lipid and glycemic profile reasonable, particularly in men with functional hypogonadism or metabolic risk factors.

Liver and Kidney Markers

A comprehensive metabolic panel may be included based on the patient’s medications, formulation and health history.

Monitoring requirements can differ between injectable, topical and oral testosterone products.

When Should TRT Be Adjusted or Paused?

TRT may need to be adjusted, temporarily held or reconsidered when monitoring shows:

  • Hematocrit above 54%
  • Persistent blood pressure elevation
  • Testosterone levels consistently above the intended range
  • Significant acne or mood changes
  • Worsening sleep apnea
  • New or worsening leg swelling
  • Significant fluid retention
  • Worsening urinary obstruction
  • A concerning PSA change
  • No meaningful symptom improvement
  • A major change in fertility goals
  • A new cardiovascular or clotting event

The response depends on the specific problem.

A high hematocrit does not always require permanently stopping therapy. The provider may reduce the dose, change the delivery method, evaluate sleep apnea or temporarily pause treatment while investigating contributing factors.

Similarly, a PSA increase requires confirmation and appropriate evaluation. It should not be ignored, but it also should not be treated as an automatic cancer diagnosis.

Warning Signs That Need Prompt Medical Attention

Contact your healthcare team promptly for:

  • Repeatedly elevated blood pressure readings
  • Severe or persistent headaches
  • Rapidly worsening swelling
  • New difficulty urinating
  • Significant changes in mood or behavior
  • Worsening sleep apnea symptoms
  • A laboratory result showing markedly elevated hematocrit
  • New breast lumps or significant breast tenderness

Seek urgent medical care for:

  • Chest pain
  • Sudden shortness of breath
  • Coughing up blood
  • One-sided leg swelling or pain
  • Sudden weakness or numbness
  • Difficulty speaking
  • Sudden vision changes
  • Severe blood pressure elevation with concerning symptoms

Blood pressure above 180/120 mm Hg with symptoms such as chest pain, breathing difficulty, weakness or vision changes may represent a hypertensive emergency.

Common TRT Monitoring Mistakes

Only Checking Total Testosterone

Total testosterone matters, but it does not replace monitoring hematocrit, blood pressure, symptoms and prostate risk.

Drawing Labs at Random Times

A result taken at a peak cannot be accurately compared with a previous trough result.

Lab timing should remain consistent relative to the dose.

Adjusting the Dose Too Quickly

Symptoms and hormone levels may need time to stabilize.

Frequent dose changes can create wider fluctuations and make it harder to identify what is helping or causing side effects.

Ignoring Sleep Apnea

Untreated sleep apnea can worsen fatigue, blood pressure and elevated hematocrit.

Increasing testosterone does not correct poor breathing during sleep.

Treating Estradiol Automatically

Estradiol should not be suppressed simply because it appears above a preferred number.

Symptoms, testosterone levels, body composition and the full clinical picture matter.

Using Blood Donation as the Entire Plan

Repeated donation may lower hematocrit temporarily. It does not correct an excessive dose, sleep apnea, smoking, respiratory disease or another underlying contributor.

Continuing Treatment Without Symptom Improvement

If appropriately timed testosterone levels normalize but the patient still feels no better, the provider should reconsider other causes.

The original problem may involve sleep, thyroid function, depression, medication effects, insulin resistance, cardiovascular disease or another health condition.
