Testosterone for Women in Menopause: What We Know and What We Still Don’t

Testosterone is often thought of as a male hormone.

But women produce testosterone too.

And during midlife, questions about testosterone are becoming increasingly common:

“Could low testosterone be causing my low libido?”

“Will testosterone help my energy?”

“Can it help me maintain muscle?”

“Should testosterone be part of menopause hormone therapy?”

The answer is more nuanced than many social media posts make it sound.

There is meaningful evidence supporting testosterone therapy for a specific group of postmenopausal women, particularly those experiencing hypoactive sexual desire disorder (HSDD).

But the evidence becomes much less certain when testosterone is promoted for energy, weight loss, brain fog, mood, muscle building, or general “hormone optimization.”

Current international consensus guidance states that the strongest evidence-based indication for testosterone therapy in women is treatment of HSDD in appropriately evaluated postmenopausal women. Long-term safety and many other proposed benefits remain less certain.

Here is what we currently know, what we do not, and why the distinction matters.

Women Naturally Produce Testosterone

Testosterone is not exclusively a male hormone.

In women, androgens are produced through the ovaries and adrenal glands and through conversion of other hormones in peripheral tissues.

Testosterone interacts with androgen receptors throughout the body and may influence several biological systems, including sexual function.

Hormone production and circulating androgen levels change throughout adulthood. Surgical removal of the ovaries can cause a particularly abrupt change in androgen production.

But an important distinction is often overlooked:

A low testosterone number does not automatically mean a woman has “testosterone deficiency.”

There is currently no universally accepted blood testosterone cutoff that diagnoses sexual dysfunction in women.

International consensus recommendations specifically state that circulating testosterone levels cannot reliably distinguish women with sexual dysfunction from women without it.

That means treatment should not begin simply because a laboratory report flags a testosterone result as “low.”

Symptoms, medical history, medications, relationship factors, physical health, menopause status, sexual function, and other hormones all matter.

The Strongest Evidence: Testosterone and Low Sexual Desire

The clearest evidence for testosterone therapy in women involves hypoactive sexual desire disorder, commonly shortened to HSDD.

HSDD involves persistently reduced sexual desire that causes meaningful personal distress and cannot be better explained by another condition, medication, relationship issue, or other contributing factor.

Several randomized controlled trials have studied testosterone therapy in postmenopausal women with HSDD.

Research has found improvements in areas such as:

  • Sexual desire
  • Sexual responsiveness
  • Arousal
  • Orgasmic function
  • Sexual pleasure
  • Sexual satisfaction
  • Distress related to low sexual desire

The 2019 Global Consensus Position Statement concluded that physiologic testosterone therapy produces a moderate improvement in sexual function among appropriately selected postmenopausal women with HSDD.

A 2026 systematic review evaluating 33 studies also found that the strongest evidence continues to come from postmenopausal women, with randomized trials demonstrating improvements in libido and sexual-function outcomes.

What does “moderate benefit” actually mean?

Testosterone is not a guaranteed libido switch.

Across the research, women receiving physiologic testosterone experienced improvements compared with placebo, but results varied between individuals.

Some women respond meaningfully.

Others may notice little or no improvement.

That is one reason appropriate diagnosis and follow-up matter.

Low Libido Is Not Always a Testosterone Problem

This may be one of the most important parts of the discussion.

Sexual desire is influenced by far more than testosterone.

Low libido during menopause may be affected by:

  • Falling estrogen levels
  • Vaginal dryness
  • Pain during intercourse
  • Genitourinary syndrome of menopause
  • Sleep disruption
  • Hot flashes
  • Depression
  • Anxiety
  • Chronic stress
  • Relationship changes
  • Certain antidepressants
  • Other medications
  • Thyroid dysfunction
  • Chronic illness
  • Pelvic-floor dysfunction
  • Body-image changes
  • Fatigue
  • Alcohol use

That is why current sexual-health guidance recommends a biopsychosocial evaluation rather than diagnosing HSDD from a hormone panel alone.

For example, a woman may describe “low libido” when the underlying issue is painful intercourse caused by estrogen-related vaginal changes.

Treating the pain and tissue changes may improve sexual function without testosterone.

Another woman may have severely disrupted sleep and high stress.

Another may have started a medication known to affect sexual desire.

The goal should be to determine what is actually interfering with sexual function before choosing treatment.

Can Testosterone Improve Energy in Menopausal Women?

This is where the evidence becomes much weaker.

Fatigue is commonly marketed as a symptom of “low testosterone” in women.

But current clinical evidence does not establish testosterone as a reliable treatment for general fatigue or low energy in postmenopausal women.

The Global Consensus Position Statement found insufficient evidence to support testosterone for general wellbeing, and available controlled data did not demonstrate a clear overall wellbeing benefit.

Fatigue during menopause can have many causes, including:

  • Poor sleep
  • Sleep apnea
  • Iron deficiency
  • Thyroid dysfunction
  • Calorie or protein restriction
  • Depression
  • Chronic stress
  • Insulin resistance
  • Medication effects
  • Menopause-related sleep disruption
  • Nutrient deficiencies
  • Excessive training with inadequate recovery

Testosterone should not become a shortcut that prevents those possibilities from being investigated.

What About Testosterone for Brain Fog and Cognition?

Brain fog is another symptom frequently associated with menopause.

Women may experience problems with:

  • Concentration
  • Word recall
  • Mental stamina
  • Working memory
  • Focus

But testosterone has not been established as a treatment for menopausal cognitive symptoms.

International consensus guidance concludes that there is currently insufficient evidence showing testosterone improves cognitive performance or prevents cognitive decline in postmenopausal women.

A 2025 review in Obstetrics & Gynecology similarly concluded that current evidence does not support testosterone for improving cognition or brain health in women.

Sleep, vasomotor symptoms, mood, thyroid function, medications, stress, and other factors may deserve evaluation when brain fog develops during menopause.

Does Testosterone Help Women Build Muscle?

This is another area where expectations often exceed the evidence.

Testosterone clearly affects muscle physiology.

That does not automatically mean testosterone therapy has been proven to preserve or build muscle in otherwise healthy menopausal women.

Current consensus recommendations state that evidence is insufficient to prescribe testosterone specifically for:

  • Increasing lean body mass
  • Improving muscle strength
  • Preventing age-related muscle loss
  • Improving bone density
  • Preventing fractures

More appropriately powered clinical trials are still needed to answer these questions.

For maintaining muscle during and after menopause, the strongest practical foundation remains:

  • Progressive resistance training
  • Adequate protein
  • Sufficient calorie intake
  • Good sleep
  • Recovery
  • Treating relevant medical or hormonal conditions when indicated

Testosterone should not substitute for these fundamentals.

Can Testosterone Help With Weight Loss?

Testosterone is sometimes promoted online as a metabolism or fat-loss treatment for menopausal women.

Current evidence does not support testosterone as a standard weight-loss therapy for women.

Changes in body composition during menopause can involve multiple factors:

  • Declining estrogen
  • Aging
  • Reduced activity
  • Loss of muscle mass
  • Sleep disruption
  • Insulin resistance
  • Changes in appetite
  • Reduced energy expenditure
  • Stress
  • Dietary changes

Weight or body-composition concerns deserve a broader metabolic evaluation rather than assuming testosterone is the missing piece.

Testosterone and Mood: What Do We Know?

Some women report subjective mood or motivation changes while receiving testosterone.

Individual experiences matter clinically.

But they should not be confused with established population-level evidence.

Current controlled studies have not shown consistent evidence that testosterone improves depressed mood in postmenopausal women.

That means testosterone is not currently considered a standard treatment for depression, anxiety, or generalized mood symptoms during menopause.

How Is Testosterone Given to Women?

When testosterone therapy is considered for appropriately evaluated women with HSDD, major clinical guidelines generally favor transdermal testosterone delivered in doses intended to maintain testosterone within the physiologic female range.

Transdermal therapy is applied through the skin.

One major challenge in the United States is that there is currently no FDA-approved testosterone product specifically indicated for women.

FDA-approved testosterone products are designed for men.

Therefore, testosterone use for women in the United States is generally off-label.

Off-label prescribing is common throughout medicine, but it makes appropriate dosing and monitoring particularly important.

Why Testosterone Dosing for Women Requires Precision

Women generally require substantially smaller testosterone doses than those used for men.

The goal is not to push testosterone as high as possible.

It is to maintain exposure within an appropriate physiologic female range while monitoring symptoms and potential androgen-related side effects.

International guidance recommends avoiding formulations that consistently produce supraphysiologic testosterone concentrations.

Higher is not automatically better.

Excessive androgen exposure may increase adverse effects without improving the underlying problem.

What About Testosterone Pellets?

Testosterone pellets have become popular in some hormone-optimization clinics.

However, major international consensus guidance does not recommend testosterone preparations that result in supraphysiologic concentrations and specifically cautions about pellets or injectable approaches when they produce levels above the normal physiologic female range.

One practical concern with a pellet is reversibility.

A topical medication can usually be reduced or discontinued relatively quickly.

Once a pellet has been implanted, exposure cannot be adjusted as easily.

Treatment route should therefore be discussed carefully with a qualified clinician.

What About Compounded Testosterone?

Because there is no FDA-approved testosterone product specifically formulated for women in the United States, compounded testosterone is sometimes used.

However, compounded medications are not evaluated through the same FDA approval process for consistency, safety, and effectiveness as FDA-approved products.

The FDA notes broadly that compounded hormone products are not FDA-approved and has not established that compounded “bioidentical” hormone products are safer or more effective than approved hormone therapies.

The International Society for the Study of Women’s Sexual Health recommends using approved transdermal formulations designed for men at carefully reduced female dosing when appropriate rather than routinely relying on compounded testosterone when a suitable regulated product can be used.

What Side Effects Can Testosterone Cause in Women?

When testosterone levels remain within physiologic female ranges, short-term studies generally report relatively low rates of serious adverse effects.

Androgen-related side effects may include:

  • Acne
  • Increased facial or body hair
  • Oily skin

Higher or excessive exposure may create greater concern for androgenic effects.

Potential changes such as:

  • Voice deepening
  • Scalp hair loss
  • Clitoral enlargement

deserve prompt clinical attention.

The important distinction is that the clinical trials supporting testosterone therapy generally involved physiologic dosing, not aggressive hormone optimization designed to produce high testosterone concentrations.

What About Breast Cancer Risk?

This remains one of the major unanswered questions.

Available short-term clinical trials have not established a major breast-cancer safety signal with physiologic testosterone therapy.

However, long-term evidence remains insufficient.

The Global Consensus Position Statement specifically called for longer and better-powered studies examining breast safety.

The FDA has also identified breast cancer risk as one of the important long-term knowledge gaps surrounding testosterone use in menopausal women.

That is different from saying testosterone has been shown to cause breast cancer.

It means researchers do not yet have enough high-quality long-term evidence to confidently quantify the risk.

What About Heart and Cardiovascular Risk?

The same caution applies to cardiovascular safety.

Short-term randomized trials using physiologic doses have not demonstrated major cardiovascular safety signals.

But many clinical trials excluded women with significant cardiometabolic disease, making it difficult to generalize the findings to all women.

Long-term cardiovascular safety has not been adequately established.

The FDA specifically lists cardiovascular outcomes among the areas requiring additional research.

Do Testosterone Blood Tests Matter?

Yes, but perhaps not in the way many people expect.

A testosterone level should not be used by itself to diagnose HSDD.

There is no validated testosterone concentration below which a woman automatically has sexual dysfunction.

Testing can still play an important role.

Before therapy, clinicians may assess:

  • Total testosterone
  • SHBG
  • Other hormones when clinically indicated
  • Relevant metabolic or health markers

The baseline testosterone level helps identify unexpectedly high testosterone levels and provides a reference for monitoring therapy.

Clinical guidelines recommend repeat testing after treatment begins and periodic monitoring to help prevent excessive exposure.

The treatment target is not simply “make the number higher.”

The goal is improvement in the clinically relevant symptom without producing excessive androgen exposure.

When Should Treatment Be Reassessed?

Testosterone is not something that should simply be prescribed indefinitely without evaluating whether it is working.

Consensus guidance recommends assessing:

  1. Whether sexual desire and associated distress have meaningfully improved.
  2. Whether testosterone concentrations remain within an appropriate physiologic range.
  3. Whether androgen-related side effects have appeared.
  4. Whether another condition may better explain persistent symptoms.

The Global Consensus Position Statement recommends discontinuing testosterone when there is no meaningful benefit after approximately six months.

Who Might Be a Reasonable Candidate for Testosterone?

Based on current evidence, the clearest potential candidate is a postmenopausal woman who:

  • Has persistent low sexual desire
  • Experiences meaningful distress because of it
  • Meets criteria consistent with HSDD
  • Has undergone a broader evaluation
  • Does not have a more obvious untreated cause
  • Understands that treatment is off-label in the United States
  • Is willing to undergo appropriate follow-up and monitoring

This is very different from prescribing testosterone because someone reports fatigue or because one lab result appears low.

What We Know About Testosterone for Women

Current evidence supports several conclusions.

We know women naturally produce testosterone.

It is biologically relevant to female physiology.

We know testosterone can improve sexual function in some postmenopausal women with HSDD.

This is currently the strongest evidence-based indication.

We know the treatment effect is not universal.

Some women respond better than others.

We know blood testosterone alone cannot diagnose HSDD.

Symptoms and clinical context matter.

We know physiologic dosing matters.

More testosterone is not necessarily better.

We know monitoring is important.

Testing helps identify excessive exposure and supports safer treatment.

What We Still Don’t Know

There are equally important questions that remain unresolved.

1. Long-term cardiovascular safety

We need larger and longer randomized trials.

2. Long-term breast safety

Existing evidence is not strong enough to establish long-term risk with confidence.

3. Whether testosterone meaningfully improves muscle or bone outcomes

The biological rationale is interesting, but clinical evidence remains insufficient.

4. Whether testosterone improves cognition or brain health

Current evidence does not support prescribing it for this purpose.

5. Whether testosterone reliably improves energy or general wellbeing

This has not been clearly demonstrated.

6. The optimal treatment duration

Long-term therapy has not been studied as thoroughly as short-term use.

7. Which women are most likely to respond

Better biomarkers and predictive clinical tools are still needed.

8. The best standardized female formulation

The lack of an FDA-approved testosterone formulation specifically for women remains a significant clinical gap in the United States.

Why This Question Is Getting More Attention in 2026

Interest in women’s testosterone therapy has increased substantially.

The scientific discussion is evolving as well.

A 2026 systematic review found continued evidence that testosterone can improve several measures of sexual function, with the strongest randomized trial evidence occurring in postmenopausal women. The authors also emphasized the need for more research into dosing and long-term safety.

The FDA has scheduled a public workshop for September 17, 2026, specifically focused on testosterone use in menopausal women.

The agency says important gaps remain around:

  • Sexual function
  • Cognition
  • Mood
  • Musculoskeletal health
  • Testosterone measurement
  • Cardiovascular safety
  • Breast safety

That is a useful reflection of where the science stands today.

Testosterone for women should not be dismissed.

But it should not be oversold either.

A Better Approach to Hormone Symptoms During Menopause

The most useful question is usually not:

“Is my testosterone low?”

A better question is:

“What is causing my symptoms, and which interventions have the strongest evidence for addressing them?”

For someone experiencing low libido, that might mean looking at:

  • Testosterone
  • Estrogen status
  • Vaginal and pelvic health
  • Pain
  • Sleep
  • Stress
  • Thyroid function
  • Medications
  • Mental health
  • Relationship factors
  • Metabolic health

For someone experiencing fatigue, weight changes, poor recovery, or brain fog, a different evaluation may be necessary.

That broader approach reduces the risk of forcing every menopause symptom into a single hormone explanation.

The Bottom Line

Testosterone deserves a legitimate place in the conversation about women’s health.

But the evidence needs to guide that conversation.

For appropriately evaluated postmenopausal women with distressing hypoactive sexual desire disorder, physiologic testosterone therapy can provide meaningful benefit.

That is where the evidence is strongest.

What we cannot yet say with the same confidence is that testosterone is a proven treatment for fatigue, weight gain, brain fog, depression, muscle loss, or general anti-aging.

And we still need better long-term evidence regarding breast health, cardiovascular health, dosing, treatment duration, and which women benefit most.

Hormone care should therefore be personalized without becoming speculative.

Evaluate the symptoms.

Understand the entire clinical picture.

Use the evidence.

Monitor the response.

And do not assume that raising a hormone number is the same thing as improving health.
