
Most people think about hormones only when something goes noticeably wrong.
Low libido. Fatigue. Weight gain. Brain fog. Poor sleep. Hot flashes. Loss of strength.
But hormones do much more than influence how you feel.
Testosterone, estrogen, progesterone, thyroid hormones, insulin, cortisol, and other signaling molecules participate in systems that regulate metabolism, muscle, bone, sexual function, energy utilization, body composition, sleep, and more.
That means hormone health shouldn’t be viewed in isolation.
The Question Isn’t Simply: “Are My Hormones Low?”
Aging brings predictable biological changes.
In men, testosterone concentrations often decline over time, although aging itself isn’t the only reason. Obesity, medications, chronic illness, and lifestyle factors can contribute substantially to lower testosterone levels.
In women, the transition through perimenopause and menopause involves significant fluctuations and eventual reductions in ovarian hormone production. These changes can contribute to hot flashes, sleep disturbances, vaginal symptoms, and accelerated bone loss.
But there’s another piece of the story.
Some of the health conditions that become more common as we age can themselves influence hormone levels.
This creates an important distinction:
Low hormones can sometimes be part of the problem—but they can also be a signal that something else in the body needs attention.
Testosterone and Metabolic Health: A Two-Way Street
This is particularly evident when we look at testosterone.
Research has found strong relationships between obesity, insulin resistance, metabolic syndrome, type 2 diabetes, and lower testosterone in men.
In fact, a review examining nearly 20,000 men with obesity reported that approximately 42.8% had total testosterone below 300 ng/dL. The relationship appears to be bidirectional and complex: excess adiposity and metabolic dysfunction can suppress the hypothalamic-pituitary-testicular axis, while lower testosterone can coexist with unfavorable changes in muscle mass and body composition.
This is why simply asking:
“What’s my testosterone?”
doesn’t tell the entire story.
A better question is:
“What is happening throughout my physiology that may be influencing my hormones, metabolism, body composition, and how I feel?”
Women’s Hormones Matter Beyond Hot Flashes
For women, declining estrogen during menopause isn’t simply about hot flashes.
Estrogen plays important roles throughout the body, including bone physiology. Systemic estrogen therapy has been shown to reduce the bone loss occurring early in menopause and can help prevent osteoporosis-related fractures.
But this is also where responsible medical care matters.
Hormone therapy isn’t appropriate for every woman, and the potential benefits and risks vary according to age, medical history, symptoms, timing of menopause, treatment type, and other factors.
For example, ACOG notes that menopausal hormone therapy is highly effective for vasomotor symptoms and can protect against bone loss, while certain formulations and patient populations may face increased risks such as blood clots, stroke, or certain cancers. It should not simply be prescribed as a universal strategy to prevent heart disease.
Optimization isn’t about giving everyone hormones.
It’s about understanding the individual.
Symptoms Are Signals—Not Diagnoses
Consider how many seemingly unrelated symptoms can occur together:
- declining energy
- difficulty maintaining muscle
- increased abdominal fat
- reduced libido
- poor sleep
- brain fog
- mood changes
- decreased exercise recovery
- hot flashes or menstrual changes
- difficulty losing weight
None of these automatically means someone has a hormone deficiency.
The Endocrine Society specifically emphasizes this point for men: symptoms such as low energy, libido, or mood are not enough to diagnose hypogonadism. Appropriate diagnosis requires symptoms plus consistently low, accurately measured testosterone, while clinicians should also evaluate potentially reversible contributors.
That’s exactly why testing matters.
Don’t Guess. Measure.
Modern health care often becomes reactive.
A symptom appears.
Then another.
Then another.
Eventually a medication may be prescribed for each individual problem.
There are absolutely situations where medication is necessary and lifesaving. But there is also tremendous value in stepping back and looking at the larger physiological picture.
That can include evaluating:
Hormones. Metabolic health. Thyroid function. Blood sugar regulation. Lipids. Nutrient status. Inflammation. Body composition. Lifestyle. Sleep.
The objective isn’t to chase one “perfect” number.
It’s to understand patterns.
Hormone Optimization Isn’t a Replacement for Healthy Living
This may be the most important part of the conversation.
Hormone therapy should never become a substitute for exercise, nutrition, sleep, appropriate weight management, or treatment of underlying disease.
The Endocrine Society recently reinforced this specifically regarding testosterone and metabolic health: testosterone treatment should not replace lifestyle intervention, weight management, or established diabetes-prevention strategies.
