The next, your sleep is disrupted, your period arrives early, your mood feels different, and you suddenly wonder what happened to the body you thought you understood.

This unpredictability is one of the defining features of perimenopause.

Contrary to the idea that hormones simply decline in a straight line as menopause approaches, the hormonal transition can be much more variable. Estrogen may rise, fall, and spike from one cycle to another, while progesterone production can become less consistent as ovulation becomes more unpredictable.

That helps explain why symptoms can feel intense one month and relatively mild the next.

Understanding what is happening hormonally can make this transition feel much less confusing.

What Is Perimenopause?

Perimenopause is the transitional period leading up to menopause.

After ovulation, the structure left behind in the ovary, called the corpus luteum, produces progesterone.

But during perimenopause, ovulation can become less consistent.

According to the American College of Obstetricians and Gynecologists, some months during perimenopause the ovaries release an egg, while during other months they may not.

When ovulation does not occur normally, progesterone production can be reduced or absent for that cycle.

As a result, progesterone exposure may become increasingly inconsistent even while estrogen continues to fluctuate.

That changing relationship between estrogen and progesterone can contribute to noticeable changes in the menstrual cycle.

Periods may become:

  • Closer together
  • Farther apart
  • Heavier
  • Lighter
  • Longer
  • Shorter
  • Occasionally skipped

These changes are common during perimenopause, although significant or unusual bleeding should still be evaluated by a healthcare professional.

Why Hormones Can Change So Much From Month to Month

Think of reproductive hormones before perimenopause as following a relatively organized monthly rhythm.

It is not perfectly identical every cycle, but there is usually a recognizable pattern.

As ovarian follicles decline with age and ovarian function becomes less predictable, that rhythm becomes less consistent.

One cycle might include relatively typical ovulation.

Another might involve delayed ovulation.

Hot flashes often begin during perimenopause and may become more noticeable as women approach their final menstrual period.

Sleep Changes

Poor sleep can then amplify other problems, including:

A cycle that was once reliably 28 or 30 days may suddenly become 23 days, then 34 days, then return to something closer to normal.

Flow may also change.

Why One Hormone Test May Not Tell the Whole Story

“Can’t I just get my hormones tested to see whether I’m in perimenopause?”

Sometimes hormone testing is useful, particularly when a clinician is evaluating other possible causes of symptoms.

But a single estrogen measurement only captures what was happening at that particular moment.

If hormone concentrations are fluctuating substantially from one cycle to another, tomorrow’s result could look very different from today’s.

For women in the typical age range with characteristic cycle and symptom changes, clinicians often rely heavily on:

  • Age
  • Menstrual history
  • Symptom patterns
  • Medical history
  • Other possible causes of symptoms

rather than interpreting one hormone result in isolation.

Symptoms resembling perimenopause can also overlap with thyroid disorders, medication effects, iron deficiency, sleep disorders, mental health conditions, and other medical issues.

This is one reason context matters.

Perimenopause Is Not Just “Low Estrogen”

One of the biggest misconceptions surrounding perimenopause is that every symptom means estrogen is simply too low.

The physiology is more complicated.

During parts of the transition, estrogen can still be substantial and can fluctuate considerably.

At the same time, inconsistent ovulation can change progesterone production.

Eventually, as menopause approaches, ovarian estrogen production declines more persistently.

So rather than thinking:

High hormones → gradually lower hormones → menopause

it can be more accurate to picture:

Regular hormonal patterns → increasingly unpredictable hormonal patterns → persistently lower ovarian hormone production after menopause

That distinction helps explain why the transition can feel so erratic.

When Does Perimenopause End?

Perimenopause ends when menopause is reached.

Clinically, menopause is confirmed after 12 months without menstrual bleeding, assuming there is no other medical explanation for the absence of periods.

After menopause, ovarian production of estrogen and progesterone remains substantially lower than during the reproductive years.

The unpredictable cycling characteristic of perimenopause therefore eventually gives way to a more consistently low-hormone environment.

That does not mean every symptom immediately disappears.

For example, hot flashes and other menopausal symptoms may continue beyond the final menstrual period.

Yes, particularly if symptoms seem unpredictable.
