Cortisol Testing Explained: Blood, Saliva, Urine and the Limits of Each

Cortisol testing has become increasingly popular as more people try to understand fatigue, poor sleep, weight changes, stress, and hormone health.

But there is an important distinction that often gets missed:

A cortisol result is not simply a measurement of how “stressed” you are.

Cortisol is a tightly regulated hormone that changes throughout the day. The meaning of a cortisol test depends on why the test was ordered, when the sample was collected, which type of sample was used, medications being taken, and the clinical question being investigated.

Blood, saliva, and urine testing can all provide useful information, but they measure cortisol differently and have different limitations.

Understanding those differences can help prevent a single laboratory value from being overinterpreted.

What Is Cortisol?

Cortisol is a glucocorticoid hormone produced by the adrenal glands.

Its production is regulated by the hypothalamic-pituitary-adrenal axis, often called the HPA axis.

The process works roughly like this:

  1. The hypothalamus releases corticotropin-releasing hormone, or CRH.
  2. CRH signals the pituitary gland to release adrenocorticotropic hormone, or ACTH.
  3. ACTH tells the adrenal glands to produce cortisol.
  4. Rising cortisol eventually provides negative feedback to the brain and pituitary, helping regulate further production.

Cortisol plays important roles in:

  • Blood pressure regulation
  • Blood glucose control
  • Immune and inflammatory responses
  • Energy metabolism
  • The stress response
  • Sleep-wake regulation

Cortisol is sometimes called the “stress hormone,” but that description is incomplete.

You need cortisol to function normally.

The medical concern is not that cortisol exists or temporarily rises during stress. Problems occur when cortisol production becomes pathologically high or low, or when regulation of the HPA axis is disrupted.

Cortisol Naturally Changes Throughout the Day

One reason cortisol testing can be confusing is that cortisol is not supposed to remain constant.

For most people, cortisol follows a circadian rhythm.

Levels generally rise during the early morning hours, are relatively high around waking, and gradually decline throughout the day before reaching low levels around bedtime.

That means a cortisol measurement taken at 8 a.m. cannot necessarily be interpreted the same way as one taken at 4 p.m. or midnight.

Timing matters.

So does the purpose of the test.

A clinician evaluating possible adrenal insufficiency may be interested in morning cortisol production.

A clinician evaluating possible Cushing syndrome may be more interested in whether cortisol remains abnormally elevated late at night.

This is why there is no universally “best” cortisol test.

The appropriate test depends on the clinical question.

Blood Cortisol Testing

Blood testing measures cortisol circulating in the bloodstream at the moment the blood sample is collected.

It is one of the most familiar forms of cortisol testing and can be particularly useful when evaluating suspected adrenal insufficiency.

What Blood Cortisol Can Tell You

Morning serum cortisol may provide information about whether the adrenal glands are producing an appropriate amount of cortisol.

For suspected adrenal insufficiency, clinicians commonly evaluate cortisol in the morning because cortisol levels should normally be relatively high at that time.

The Endocrine Society recommends morning serum cortisol as an initial assessment in certain evaluations of adrenal insufficiency, while an ACTH stimulation test is typically used when confirmation is necessary.

Blood cortisol can also be measured as part of specialized testing, including a dexamethasone suppression test when evaluating suspected excess cortisol.

Limitations of Blood Cortisol Testing

A blood cortisol level is essentially a snapshot.

It does not automatically tell you how cortisol behaved earlier that morning, throughout the afternoon, or during the previous night.

Blood cortisol results can also be influenced by factors such as:

  • Time of collection
  • Acute illness
  • Recent physical stress
  • Medication use
  • Changes in cortisol-binding proteins
  • Pregnancy or estrogen-containing medications
  • The specific laboratory assay being used

Another important distinction is that most standard serum cortisol testing reflects total cortisol, which includes cortisol attached to carrier proteins as well as free cortisol.

That can sometimes complicate interpretation when cortisol-binding proteins are altered.

Bottom Line on Blood Testing

Blood cortisol can be extremely useful when ordered for the right reason, particularly during evaluation of adrenal insufficiency or as part of a suppression or stimulation test.

But a random cortisol blood test taken without considering timing and clinical context often provides limited information.

Salivary Cortisol Testing

Salivary cortisol testing measures cortisol that passes from the bloodstream into saliva.

Because it primarily reflects unbound or biologically available cortisol, saliva testing can be useful in specific clinical situations.

The best-established use is late-night salivary cortisol testing when evaluating suspected Cushing syndrome.

 

Why Late-Night Salivary Cortisol Matters

Cortisol should normally become very low late in the evening.

In people with Cushing syndrome, that normal nighttime decline may be lost.

The Endocrine Society lists late-night salivary cortisol as one of the recommended initial screening options for suspected Cushing syndrome, generally using more than one measurement.

The NIDDK similarly notes that people can collect late-night saliva samples at home to evaluate whether cortisol remains abnormally elevated when it should normally be low.

 

Advantages of Saliva Testing

Potential advantages include:

  • Noninvasive sample collection
  • Collection at home
  • Ability to assess cortisol late at night
  • No needle-related stress during collection
  • Measurement of primarily free cortisol

These characteristics make saliva particularly useful when the timing of cortisol secretion matters.

Limitations of Salivary Cortisol Testing

Saliva testing is not automatically a comprehensive measurement of “adrenal health.”

Results depend heavily on appropriate sample collection.

Factors that can interfere with interpretation may include:

  • Collecting the sample at the wrong time
  • Shift work
  • Irregular sleep schedules
  • Eating or drinking shortly before testing
  • Blood contamination from the gums
  • Smoking
  • Certain medications
  • Acute stress
  • Laboratory methodology

A single sample can also miss intermittent abnormalities.

Cushing syndrome, for example, can occasionally involve fluctuating cortisol production, which is one reason multiple tests may be required.

Bottom Line on Saliva Testing

Salivary cortisol is clinically useful, especially when measuring late-night cortisol during a Cushing syndrome evaluation.

However, several saliva samples collected throughout one day should not automatically be interpreted as a definitive diagnosis of chronic stress, “adrenal fatigue,” or HPA-axis dysfunction.

Those conclusions require much more clinical context.

24-Hour Urinary Free Cortisol Testing

A 24-hour urinary free cortisol test looks at cortisol excreted in the urine over an entire day.

Rather than providing a single-moment measurement, it provides an estimate of cortisol exposure across approximately 24 hours.

What Urinary Free Cortisol Can Tell You

Urinary free cortisol is commonly used when evaluating suspected cortisol excess, particularly Cushing syndrome.

The Endocrine Society includes 24-hour urinary free cortisol among its recommended initial tests for Cushing syndrome and recommends obtaining at least two measurements when this method is used.

The NIDDK also identifies the 24-hour urinary free cortisol test as one of the primary approaches used to diagnose Cushing syndrome.

Advantages of 24-Hour Urine Testing

One of its major advantages is that it integrates cortisol production over a longer period.

Instead of asking:

“What was cortisol at 8 a.m.?”

It asks something closer to:

“How much free cortisol was excreted across this entire day?”

That can be useful when clinicians are looking for sustained cortisol excess.

Limitations of Urinary Cortisol Testing

The test depends heavily on accurate collection.

Every urine sample during the collection period generally needs to be captured according to the laboratory’s instructions.

Missing samples can make the result unreliable.

Other limitations may include:

  • Incomplete urine collection
  • Very high or very low fluid intake
  • Kidney dysfunction
  • Day-to-day cortisol variability
  • Certain medications
  • Mild or intermittent cortisol excess

The Endocrine Society specifically notes that urinary free cortisol may be less appropriate in people with severe renal failure.

Bottom Line on Urine Testing

A 24-hour urinary free cortisol measurement can be useful for identifying overall cortisol excess.

It is less useful for determining exactly when during the day cortisol is abnormal, because the entire day’s production is combined into one measurement.

Blood vs. Saliva vs. Urine Cortisol Testing

Each method answers a slightly different question.

The important takeaway is that these tests are not interchangeable.

A clinician chooses a test based on the suspected condition and the physiology that needs to be measured.

Why One Cortisol Test Rarely Tells the Whole Story

Cortisol is affected by more than adrenal gland function.

Short-term changes may occur in response to:

  • Illness
  • Sleep loss
  • Exercise
  • Psychological stress
  • Pain
  • Calorie restriction
  • Changes in sleep timing
  • Medications
  • Alcohol intake

This means an abnormal result does not automatically identify the cause.

When clinicians investigate genuine disorders of cortisol production, they generally look at the pattern of findings rather than relying on one isolated value.

For suspected Cushing syndrome, the Endocrine Society recommends validated screening strategies and further testing when initial findings are abnormal. Concordantly abnormal results on two different tests may lead to additional evaluation to determine the cause.

The NIDDK similarly notes that no single test is perfect and that clinicians often need more than one test to confirm Cushing syndrome.

What About Four-Point or “Cortisol Curve” Saliva Tests?

Some functional and wellness testing approaches measure saliva multiple times throughout the day, such as:

  • Shortly after waking
  • Around noon
  • Late afternoon
  • At bedtime

The idea is to create a picture of cortisol’s daily rhythm.

Physiologically, measuring cortisol at different times can demonstrate how levels vary during that particular day.

But interpretation requires caution.

A daily cortisol curve may be affected by:

  • What time the person woke up
  • Recent sleep quality
  • Exercise
  • Meals
  • Work schedule
  • Acute emotional stress
  • Caffeine
  • Medications
  • Sample timing
  • Sample collection technique

A curve that looks unusual does not independently prove that the adrenal glands are diseased.

This distinction matters because cortisol patterns are sometimes marketed as proof of conditions such as “adrenal fatigue.”

Major endocrine organizations do not recognize adrenal fatigue as an established medical diagnosis.

Symptoms attributed to it, including fatigue, sleep problems, brain fog, and changes in energy, can occur for many different reasons and deserve a broader medical evaluation.

What Cortisol Testing Cannot Tell You by Itself

Cortisol testing cannot independently diagnose the cause of:

  • Fatigue
  • Weight gain
  • Anxiety
  • Poor sleep
  • Brain fog
  • Low motivation
  • Exercise intolerance
  • Afternoon energy crashes
  • Difficulty losing body fat

These symptoms may involve multiple systems.

For example, persistent fatigue could potentially relate to:

  • Thyroid dysfunction
  • Iron deficiency
  • Sleep apnea
  • Poor sleep quantity
  • Medication effects
  • Depression or anxiety
  • Undereating
  • Excessive training
  • Chronic illness
  • Blood glucose abnormalities
  • Hormonal changes
  • Adrenal disorders

A cortisol test can be one piece of the evaluation.

It should not automatically become the entire explanation.

When Doctors Test for High Cortisol

Pathologically elevated cortisol over time can occur in Cushing syndrome.

Potential clinical features may include progressive combinations of:

  • High blood pressure
  • High blood glucose
  • Muscle weakness
  • Easy bruising
  • Bone loss
  • Changes in body-fat distribution
  • Wide purple stretch marks
  • Menstrual changes

Because common symptoms such as fatigue and weight gain occur in many conditions, widespread cortisol screening is not recommended for everyone.

The Endocrine Society recommends testing primarily when clinical features meaningfully raise suspicion for Cushing syndrome.

When testing is appropriate, validated screening approaches include:

  • Late-night salivary cortisol
  • 24-hour urinary free cortisol
  • Overnight dexamethasone suppression testing

When Doctors Test for Low Cortisol

Low cortisol can occur in adrenal insufficiency.

Possible symptoms can include:

  • Severe or persistent fatigue
  • Weakness
  • Low blood pressure
  • Unintentional weight loss
  • Nausea
  • Abdominal symptoms
  • Salt cravings in some forms of adrenal insufficiency

Clinicians may begin with morning cortisol and ACTH measurements.

When further confirmation is needed, the ACTH stimulation test is commonly used.

During the test, synthetic ACTH is administered and cortisol is measured afterward to determine whether the adrenal glands respond appropriately. The NIDDK describes the ACTH stimulation test as the test used most often to diagnose adrenal insufficiency.

Cortisol Testing Should Answer a Clinical Question

Before ordering cortisol testing, the most useful question is not:

“Which cortisol test is best?”

It is:

“What are we trying to determine?”

For example:

Suspected adrenal insufficiency:
Morning serum cortisol, ACTH, and potentially an ACTH stimulation test may be appropriate.

Suspected Cushing syndrome:
Late-night salivary cortisol, 24-hour urinary free cortisol, or dexamethasone suppression testing may be appropriate.

General fatigue or poor sleep:
A broader evaluation may be more informative than immediately assuming cortisol is the primary problem.

Testing becomes much more valuable when it is tied to a clear clinical hypothesis.

Frequently Asked Questions About Cortisol Testing

What is the most accurate cortisol test?

There is no single cortisol test that is best for every situation.

The appropriate test depends on whether a clinician is investigating cortisol excess, cortisol deficiency, or a specific feature of the daily cortisol rhythm.

Is blood or saliva better for cortisol testing?

Neither is universally better.

Blood testing is commonly used in adrenal insufficiency evaluation and during stimulation or suppression testing.

Late-night saliva testing is particularly useful for evaluating whether cortisol is appropriately suppressed at night when Cushing syndrome is suspected.

Is a 24-hour urine cortisol test more accurate?

It answers a different question.

A 24-hour urinary free cortisol test estimates free cortisol excretion over an entire day and can be useful in evaluating cortisol excess.

It does not show the timing of cortisol secretion throughout that day.

Can stress make cortisol testing abnormal?

Yes.

Cortisol responds to physiological and psychological stress.

Acute illness, sleep disruption, pain, intense exercise, and other stressors may influence cortisol concentrations.

That is one reason clinicians interpret results within the patient’s broader clinical context.

Can cortisol testing diagnose chronic stress?

Not by itself.

Cortisol physiology is complex, and a single elevated or reduced measurement cannot reliably quantify someone’s overall level of psychological stress.

Can cortisol testing diagnose adrenal fatigue?

“Adrenal fatigue” is not an established endocrine diagnosis.

Symptoms associated with the term should be evaluated for recognized medical, sleep, nutritional, psychological, medication-related, and lifestyle causes rather than being diagnosed from a cortisol profile alone.

Why would my doctor order more than one cortisol test?

Because cortisol production changes throughout the day and some disorders can be mild or intermittent.

Multiple measurements or different types of testing can improve diagnostic confidence.

Cortisol testing can provide valuable information, but only when the test matches the clinical question.

Blood cortisol provides a moment-in-time measurement.

Each has strengths.

The goal should not simply be to determine whether cortisol is “high” or “low.”

The goal is to understand why cortisol is being measured, what physiological question the test can answer, and whether the result makes sense in the larger clinical picture.
